Required Document:
Official Ayushman Bharat Card PDF or downloaded digital card screenshot clearly showing your PMJAY ID and QR code.
Upload Instructions:
Ensure the uploaded PDF is downloaded directly from the official portal (beneficiary.nha.gov.in) with no cropped edges or obscured QR codes.
